• Change of address - to be completed when you move home and still want to claim
• Change in Circumstances - to be completed to report any changes in circumstances
• Certificate of Earnings - form to be completed by employers about your earnings
• Self-Employed Earnings - form to complete detailing your business income and expenses 
• Payment on 2 Homes - application for benefit on 2 homes (only paid in special circumstances)
• Temporary Absence from home - form to complete if you will be away from your home temporarily
• Rent Proof Verification - form for Landlords to verify your rent charge to us
• Backdating Request  -  request backdating of your benefit (only paid if good cause is proven)
• Discretionary Housing Payment - form to ask for extra help towards your rent or Council Tax  
• Appeals - form to complete if you disagree with a decision we have made
• Additional Children - form where more than 3 children in the household
• Local Housing Allowance Payment to landlord - Tenant request for payment to landlord due to personal circumstances
• Local Housing Allowance Payment to landlord - Tenant request for payment to landlord because of 8 weeks rent arrears
• Local Housing Allowance Payment to landlord - Landlord request for payment because of 8 weeks rent arrears 
• Tenancy details for people over 60
