Request a Sharps Collection

Please use this form if you need to request a sharps collection. This service is free of charge.

Please ensure that your sharps boxes are:

Fully sealed
In a visible and accessible location outside your property
Not within a garage or shed
Presented by the end of the day that you are submitting this request

We will not contact you on the day of collection.

Before you start

You will need the following information to complete this form:

You should only submit the request when you have at least 3 boxes to collect, unless it is your last collection.
We cannot collect from commercial properties, including care homes.
We can only collect sharps boxes provided to you by your GP or pharmacy.
All sharps boxes must be sealed. If they are not sealed properly, we will not collect them.

Please note:

There is a time limit on each page of this form of 1 hour. Select the 'Next' / 'Submit' button on each page within an hour, otherwise your data will be lost.
All fields marked with an asterisk (*) must be completed.
